    Hi Coding and Billing KEG, 

    Hope I can get some clarity on the following question / situation. 

    A practitioner (APP or Physician) at the end of their shift leaves the critical care unit in order to document in the medical record (EHR). This location could be in an office a block away, at home or in some other quiet spot that allows the practitioner has provided critical care (not immediately available).  

    Question: With the changes in E/M documentation requirements for critical care billing (99291 / 99292) would it be permissible for the documenting practitioner to include in the total critical care time the time spent documenting from that location. 

    Any resources would be helpful.
