I am currently working on creating an adjusted heparin drip protocol for therapeutic anticoagulation in patients with intracranial hemorrhage and I wanted to see if anyone has a protocol like this at their institution that they would be willing to share? We feel that our standard therapeutic heparin drip is a bit too aggressive for patients with ICH but some of the other protocols we have rarely achieve a therapeutic PTT so we are trying to find a middle ground for these patients who need therapeutic anticoag but are also high risk for bleeding. I did a brief lit search and really didn't find anything so I wanted to reach out to see if anyone else has any experience with this? Any information you are willing to provide would be helpful!
