We at SLHS TCC have led an initiative to make the process autonomous and not dependent on the model of ICU, being closed with TCC or open with TCC. We created order sets and work flows to make ABCDE bundle part of Daily Ventilator Rounds. We also also created a flow sheet where nurses can and should document once every shift on all the elements of the bundle. Such documentation is easily viewed by all team members. It is usually documented after rounding with the intensivists who will round on these items with RN, RT, and house staff. We also incorporated the elements of the bundle into daily progress notes templates. Finally we built everything in a way we can run reports to follow on compliance with bundle elements. Also we published couple of research abstracts. At the end of the day, the TCC team check on bundle elements with the nurse and fill in on any missing components, which became minimal over time. below please see examples of our documentations and the citation.
