  • 1.  Intensivist compensation models (including RVU's)

    CCO Friends,

    The Critical Care Institute at Keck Medicine of USC is exploring Department-based models for intensivist compensation, increasingly based on RVU's.  The mantra/motivation is "equal pay for equal work", no small feat in the CCO world.  We'd appreciate the opportunity to talk to you if you're interested in sharing your experiences as they might help inform ours.  Topics we're discussing currently include 1) the role that the primary department plays (say, time in the OR [surgery, anesthesiology] vs. time in the clinic or on inpatient services [neurology, medicine]), 2) the contribution of shared billing time with APP's (such as, considering the two in combination, in other words faculty + APP revenue), 3) how does dedicated administrative time factor in (PD, Division Chief, etc), and 4) how to create incentives for investigators?  Please advise. Thanks!

    We recently went through a process that has buy-in from clinicians and administration. At its core, our CCO at Mayo manages our practice using time as the common factor. An individual commits to a portion of their FTE to critical care and this translates to clinical time. The parent department determines what happens to the same individual during the portion of FTE assigned to that department. Non-clinical time is backed out of the clinical commitment in a proportional manner. This addresses the vast majority of people/situations. When issues develop, they are adjudicated between the CCO and individual departments. This has become a transparent way for us to ensure our members are not disadvantaged by having parallel schedules. In terms of linking finances to our model of care or compensation/recognition, we are blessed that our institution recognizes what we bring and how we amplify the productivity of those around us as well as the safety/quality measures we facilitate. That is not to say we don't pay attention to documentation and finances, but we don't use it as a leverage point within.
