    - How do your teams debrief the performance of a code/RRT? 
         - Are you included in the Bedside debriefs?
         - Does anyone use video recording/playback (under QI protections)?

    We help run code blues, we don't cover medical floors so we are not involved in Rapid Response. We write a note at teh end of code blue, we don't do debriefs, which is necessary I think. Recently there was miscommunication about doing or not doing Hypothermia and this could have been resolved with a short debrief.

    Our TCC nurses do all the documentation for iCU codes in Code narrator. They also document time-outs for procedures. This helps to remove the burden of documentation from the bedside nurse. We have been working on a debriefing process with the group to using ZOLL Case Review.
