Remote Nurses

    Hi group! Are there any eICU's out there who have nurses working remotely? Our facility has shown interest and we wanted to see if anyone else has explored this option!

    VA Telecritical west , has core nursing group in Minneapolis. But now has three additional hub (chicago, Las Vegas and Los angles) from where nurse are working via network connection through central core in Minneapolis

    There is a difference between working from remote hubs in different locations versus working from home.  Which one are you referring to?

      The workflows will definitely be different depending on the number of beds that the teleCritical Care program covers and whether they also provide coverage for ED ICU boarders, RRTs, Covid Surge units, etc.

    Our workflows have really remained much the same with the addition of communication tools, but the eICU RNs at home are providing the exact same care with the same workflows and practices.

    In moving to decentralized processes- what type of communication tools do you all use? What are the contingency plans for downtimes? Is there always a 'group' at a centralized location which has redundant internet, power, etc. ?

    Craig,

          We always have at lest one nurse out of four in the work area. We do at times have three nurses at home, but there as I said there is always one in the centralized hub. If there is a outage of some sort that affects the nurse at home they are required to go in to the hub.

           We currently use an additional audio video tool for verbal communication between our staff in the hub and those at home. We have a 42 inch screen up in the hub and a speaker so the at home person/people can be seen and heard if there is a question or need. The nurses have a virtual huddle at the beginning of each shift. 


          We are also working on a Teams chat functionality to allow for non verbal communications between all staff.

    All of our at home staff connect directly to the Emory network via a private VPN, so there is not security concern from that perspective. They also sign a privacy and disclosure agreement with Emory before they work from home that also holds them accountable. For individuals at home that might experience a network issue, they know that they must come into the hub if their connection is unreliable for any reason.

    My eICU nurses work from home. I purchased laptops, monitors, a docking station and web camera for each. I currently have 5 nurses out of 15 working from home at a time (some did not want to participate). They work one month at home than two months in the clinical operations room. That way it gives everyone a chance to take advantage of this model of care. I am presenting a session at the SCCM conference to explain in more depth on Feb 8th at 9am.     The remote nurses must set up the equipment at their home. We put together an extensive instruction manual that is included with the work from home (WFJ) package to assist them. Our eICU nurses tend to be very technically friendly and have managed this very well. Once they set up the equipment they communicate with the COR and take a full assignment just like the nurses in the COR. They provide 24/7 monitoring, video rounding, best practice rounding, admission, discharge, intervention of patients they detect are decompensating from the expected trajectory and escalating to the bedside team and intensivist that works in our area.

    We are exploring this for physicians and nurses.  I'm curious as to how your Information Systems/IT department is facilitating equipment maintenance for the 5 remote positions that are being rotated?  Do the next 5 nurses get the equipment shipped to them or does the previous month's nurse send the equipment back to IT and IT sends equipment to the next nurse?  Do they set/install the equipment at each nurse's home?

    I actually own the equipment. The nurses bring the equipment back to the COR and the next nurse in the rotation comes and picks it up. Between each rotation each nurse has to test the equipment before taking it home to ensure all applications we use are functioning correctly. It does provide updates and support if there are any issues, but we have primary ownership.
